What is chronic care management?

Chronic care management is a medical program for Medicare Part B participants who have two or more chronic health conditions. Medicare defines a chronic condition as one that you expect to last for at least 12 months.

Almost all people in chronic care management are 65 or older, as that’s the standard age for Medicare enrollment. But you may qualify to enroll earlier if you receive disability benefits. As your primary care practice, Family CareX can provide chronic care management.

There’s not a specific list of allowable conditions under the Medicare chronic care management program; any chronic condition can qualify. Some of the most common are:

• In-person appointments at least once a year
• Regular remote check-ins with your medical providers (usually by phone)
• Medication management, like refills and medication adjustments
• Nutrition services and other health coaching
• Risk factor management to help you avoid injuries and health complications
